Comportamiento del sangramiento digestivo alto en adultos mayores

Abstract

La hemorragia digestiva constituye una elevada morbimortalidad en los pacientes adultos mayores de nuestro medio en el último trienio, dada su alta incidencia se decidió realizar un estudio prospectivo descriptivo de corte transversal que abarcó del mes de Enero a Dic. del 2011 con el objetivo de identificar los sitios principales de origen del sangramiento y los factores agresivos presentes . El Universo estuvo constituido por 272  adultos mayores procedentes del servicio de urgencias y consultas  y reunieron los criterios de inclusión 202, a todos se les realizó gastroduodenoscopia y se obtuvieron datos acerca de la edad, manifestaciones clínicas, sitio de sangrado hábitos tóxicos e ingestión de medicamentos. Los datos se procesaron utilizando los por cientos como medida de resumen.  Encontramos que el grupo etario de mayor edad estuvo entre 60 y 74 años; el café, la ingestión de A.I.N.E.S. la aspirina y el hábito de fumar estuvieron presentes en gran cantidad de pacientes. Los principales signos clínicos fue la hematemesis y la melena y los sitios de inicio de la hemorragia fueron varios  Concluimos que el sitio más frecuente donde se originó la hemorragia digestiva fue la gastritis erosiva y los factores agresivos más representativos fueron el café la ingestión de aspirina y A.I.N.E.S.
